The Business Model โ€” Straight Talk

How you actually earn

We believe you deserve the full, honest picture before you apply. Here is exactly how the model works and what you can โ€” and cannot โ€” expect.

๐Ÿ’ผ

You are a business owner

Every role is a 1099-NEC independent contractor position โ€” not a Wโ€‘2 job. You own your EMR and patient data, control your schedule, and are responsible for your own taxes, insurance, and business expenses. There is no salary and no benefits; there is upside you control.

๐Ÿ“ˆ

You are paid per encounter

Income is tied directly to the volume and complexity of the encounters you complete โ€” roughly $120โ€“$600 per appointment depending on service type. More panel engagement and more completed encounters mean more income. Slow weeks mean less. That is the trade for autonomy.

๐Ÿค

We supply the hard parts

Patients, a HIPAA-compliant AI-enabled platform, EMR/e-prescribing, credentialing support, malpractice coordination, training, and a multi-specialty referral network. You bring your license and your work ethic; we remove the startup friction.

Realistic 1099 earning scenarios

Based on a managed 200โ€“500 patient panel with a mix of chronic care management, preventive, and coaching encounters.

Weekly (20โ€“35 appts)
$2,400โ€“$12,000+
CCM, preventive & coaching mix
Monthly
$9,600โ€“$48,000+
Consistent panel management
Quarterly
$28,800โ€“$144,000+
Compounding retention
Annual
$115,000โ€“$500,000+
Top performers exceed this

An honest disclaimer. These figures are illustrative potential, not a promise. Actual reimbursement varies by payer, region, specialty, and โ€” most of all โ€” how much you work. Your income is not guaranteed. It is a direct function of the encounters you complete and the panel you retain. If you are looking for a fixed paycheck, this is not the right opportunity.

Who Can Apply

Find your track by credential

GoTo Telemed is built for the entire U.S. care team โ€” licensed prescribers, therapists, allied health professionals, and certified support & admin staff. Type your credential or filter by track. Every credential below maps to a specific role with a defined scope. All work is performed within your state scope of practice, and coordination roles operate under the general supervision of licensed physicians and advanced practitioners, consistent with CMS "clinical staff" and incident-to rules.

Don't see your exact credential? Apply anyway and select "Other" โ€” our credentialing team maps equivalent state licenses, national certifications, and international-to-US bridge credentials (e.g., internationally educated nurses with active US licensure) to the closest track. Every role is bounded by your state scope of practice: only licensed prescribers diagnose and prescribe; clinical coordination staff work under general supervision of licensed practitioners; and certified support & admin staff handle navigation, education, engagement, and operations โ€” never independent clinical judgment.

Why There Is a Fee โ€” And Why It Matters

The $890 one-time investment, explained

We are direct about this: launching a compliant, credentialed digital practice normally costs thousands and takes months. The one-time fee is what makes a self-operatable practice possible in weeks instead.

What the $890 covers

  • โœ… State-specific accreditation so you can practice legally in each jurisdiction.
  • โœ… Malpractice insurance with occurrence + tail coverage โ€” you are protected for services rendered.
  • โœ… External compliance verification and primary-source credentialing (CAQH, NPI, PECOS, payer enrollment support).
  • โœ… Platform setup โ€” EMR/EHR, e-prescribing, scheduling, secure messaging, and AI tooling.
  • โœ… Patient allocation โ€” a guaranteed minimum panel/encounter allocation aligned to your specialty.

Why it matters to you

Credentialing, malpractice tail coverage, and multi-state accreditation typically run into the thousands of dollars and 90โ€“180 days when done alone. The fee bundles that into one predictable, one-time cost โ€” and unlike an employer, you keep the asset: your EMR, your patient data, and your practice.

You are not out-of-pocket for internal ramp-up. On payment we credit your account with a $5,000 wallet credit for platform fees, training extensions, and marketing support. It is a promotional grant โ€” no cash value, non-transferable, expires 12 months after your effective date, and you are never billed for it.

Job 1 ยท For Nurses, Allied Health & Certified Care-Team Professionals

Care Coordinator โ€” Independent Contractor

Operate a supervised digital practice, own a 200โ€“500 patient panel, and bridge multispecialty care with AI-augmented tools โ€” under the direction of licensed physicians and nurse practitioners. Open to a wide range of licensed clinical staff and certified non-licensed support & admin professionals, each within a clearly defined tier and scope.

๐Ÿฉบ Role overview

The Care Coordinator is the linchpin of GoTo Telemed's longitudinal, digital-first care model โ€” bridging the gap between infrequent clinician encounters with continuous engagement, medication adherence, coaching, and proactive risk management. You work autonomously while following evidence-based protocols and escalation pathways. You do not diagnose or prescribe; all care plans are reviewed by licensed supervisors, and every activity is bounded by your tier and your state scope of practice.

๐Ÿ‘ฅ Who this role is for โ€” two tiers, one team

Following CMS "clinical staff" and incident-to conventions, the Care Coordinator role is structured in two tiers. Your tier is assigned at credential verification and determines which activities (and which billable encounter types) you can perform.

Tier 1 โ€” Licensed & Clinical Staff Clinical time counts toward CCM/PCM/BHI encounters

Perform protocol-driven clinical coordination under general supervision of licensed physicians/NPs: symptom check-ins, care-plan reinforcement, medication adherence review, RPM data review, behavioral health integration support, and clinical documentation. RNs may additionally update care plans within scope; LPN/LVN and MA activities are protocol-bounded per state law.

Tier 2 โ€” Certified Support & Admin Navigation, education, engagement & operations

Own the non-clinical backbone of the panel: appointment scheduling and reminders, intake and records collection, SDOH screening and community-resource navigation, health education using approved materials, wellness & lifestyle coaching (self-pay programs), patient engagement outreach, documentation support, and billing/coding operations. No independent clinical judgment, triage decisions, or medical advice โ€” clinical questions are routed to Tier 1 or supervising clinicians.

APRNs (NP, CNS, CNM, CRNA) qualify for the Telehealth Practitioner track and may also elect coordination work. Tier 2 compensation is per completed task/session at rates below the clinical encounter range; exact rates are defined in your SLA.

๐Ÿ“‹ Core responsibilities

  • Panel ownership: Manage a panel of 200โ€“500 patients, building relationships and trust.
  • Clinical support: Conduct check-ins, review symptom trackers, provide education, and reinforce care plans.
  • Supervision & escalation: Work under supervising physicians/NPs; immediately escalate any red-flag symptoms or deviations.
  • Documentation: Maintain accurate, timely SOAP notes in the GoTo Telemed EMR using templates and AI assistance (within 24 hours).
  • Coordination: Facilitate referrals, schedule follow-ups, and ensure closed-loop communication.

  • Tier 1: Active, unrestricted license (RN, LPN/LVN, etc.) or nationally recognized clinical certification (CMA/RMA/CCMA, NRP, RRT, CCM) in good standing, verified via primary source.
  • Tier 2: Nationally or state-recognized certification (CNA, CHW, NBC-HWC, CPS, CPhT, CPC, RHIT, etc.) or 2+ years of verifiable healthcare support/admin experience.

  • CMS billing & compliance: telehealth coding, documentation, fraud & abuse laws.
  • TRAN operational modules + internal GoTo Telemed orientation, tier-specific scope training.
  • 20+ hours of live shadowing, competency exam, and ongoing annual CE.

Job 2 ยท For Physicians & Advanced Practice Providers

Telehealth Practitioner โ€” Independent Contractor

Deliver video consultations, diagnose and prescribe, and supervise a coordinated care team on a HIPAA-compliant, AI-enabled platform โ€” with a guaranteed minimum patient allocation and full ownership of your EMR and patient data.

1099 Independent Contractor MD ยท DO ยท NP ยท PA ยท CNS ยท CNM ยท CRNA 200+ encounters / month You own your EMR $890 one-time investment

๐Ÿฉบ Role overview

Telehealth Practitioners are the licensed clinical backbone of GoTo Telemed. You conduct synchronous and asynchronous visits, manage diagnoses and prescriptions, and provide the clinical authority that supervises Care Coordinators. You retain sole discretion over clinical decisions and scheduling as an independent contractor โ€” GoTo Telemed supplies the platform, patients, and back office.

๐Ÿ‘ฅ Who this role is for

  • Physicians (MD / DO) โ€” primary care, specialty, and psychiatry.
  • Nurse Practitioners (NP / APRN) โ€” FNP, AGNP, PMHNP, ACNP, PNP, WHNP โ€” within state scope of practice (full, reduced, or restricted-practice states supported via collaborating arrangements).
  • Physician Assistants / Associates (PA / PA-C) โ€” with collaborating-physician arrangements.
  • Clinical Nurse Specialists (CNS) โ€” CMS-recognized billing practitioners for care management within scope.
  • Certified Nurse-Midwives (CNM) โ€” virtual prenatal/postpartum, lactation, and women's health visits within scope.
  • Certified Registered Nurse Anesthetists (CRNA) โ€” pre-anesthesia tele-assessments, chronic pain management, and coordination consults where state scope permits; CRNAs may also elect the Care Coordination track.
  • Similar advanced clinical profiles credentialed for telehealth.

โญ Supervising Physician track

Experienced physicians can take on a Supervising Physician role โ€” providing clinical oversight for Care Coordinators and, where required, collaborating-physician coverage for NPs/PAs. Supervising Physicians review care plans, hold final clinical authority over AI-surfaced recommendations, and participate in OPPE/FPPE where hospital privileges apply. This is an additional revenue stream layered on top of your own encounter volume.

๐Ÿ“‹ What we provide (per your SLA)

  • Telehealth platform: HIPAA-compliant, AI-enabled โ€” video, scheduling, EHR, e-prescribing, patient engagement.
  • Patient allocation: Guaranteed minimum of 200 patient encounters per month, aligned to your specialty and availability.
  • Credentialing support: NPI, state licensure (incl. Interstate Medical Licensure Compact), DEA, Medicare/Medicaid (PECOS), and commercial payer credentialing (CAQH).
  • EMR ownership: You own and control your complete EMR and all patient data โ€” GoTo Telemed retains no medical records beyond business-associate necessity.
  • Malpractice coordination: We coordinate with your carrier; you maintain active coverage (min. $1M / $3M).
  • Multi-specialty referral network + 24-hour initial training and annual compliance updates.

โœ… Your obligations

  • Maintain current, unrestricted licenses in every state where patients are located.
  • Carry professional liability insurance covering telehealth ($1M / $3M minimum).
  • Obtain and document informed consent for every telehealth encounter.
  • Comply with HIPAA, state telehealth law, prescribing regulations, and patient-location verification.
  • Report licensure changes, sanctions, or claims within 30 days; re-attest CAQH every 120 days.

๐Ÿ—“๏ธ Credentialing & timeline

  • Initial credentialing typically takes 90โ€“180 days from a complete application; primary-source verified (boards, NPDB, OIG, SAM).
  • Monthly monitoring of licenses, exclusions, and sanctions; full recredentialing every 3 years.
  • Setup completes within 30โ€“45 days; go-live with your allocated panel follows.
Job 3 ยท For Behavioral Health & Allied Health Professionals

Behavioral & Allied Telehealth Clinician โ€” Independent Contractor

Deliver teletherapy, counseling, nutrition, rehabilitation, speech, and medication-management services on GoTo Telemed's integrated platform โ€” with referrals flowing from the medical network, credentialing support, and complete ownership of your records and patient data. Behavioral health and allied disciplines are among the highest-utilization telehealth specialties in the U.S., and this track is purpose-built for them.

1099 Independent Contractor Psychologist ยท LCSW ยท LPC ยท LMFT ยท RD ยท PT ยท OT ยท SLP ยท PharmD Integrated referral network You own your records $890 one-time investment

๐Ÿง  Role overview

Behavioral & Allied Clinicians complete GoTo Telemed's whole-person model. Medical practitioners and care coordinators identify needs across a 200โ€“500 patient panel โ€” depression and anxiety, substance use, obesity and nutrition, post-surgical rehab, speech and swallowing, polypharmacy โ€” and route warm referrals to you inside the same platform. You deliver your discipline's services independently within your state scope of practice and license compacts (PSYPACT, Counseling Compact, Social Work Compact, PT Compact, ASLP-IC where applicable), while care coordinators handle scheduling, engagement, and follow-through.

๐Ÿ‘ฅ Who this role is for

  • Clinical Psychologists (PhD / PsyD) โ€” therapy, assessment, and testing via telehealth; PSYPACT-eligible psychologists can serve patients in 40+ states.
  • Licensed Clinical Social Workers (LCSW / LICSW) โ€” the single largest telehealth-utilizing profession in the U.S.; individual and family therapy, behavioral activation, care integration.
  • Licensed Professional Counselors (LPC / LPCC / LMHC / LCPC) โ€” talk therapy, CBT/DBT-informed care, and behavioral health integration.
  • Marriage & Family Therapists (LMFT) โ€” relationship-based and family-systems teletherapy.
  • Licensed Addiction Counselors (LADC / LISAC / CASAC / CADC) โ€” SUD counseling and recovery support alongside MAT prescribers on the practitioner track.
  • Board Certified Behavior Analysts (BCBA) โ€” telehealth ABA parent training and supervision within payer rules.
  • Registered Dietitians (RD / RDN) โ€” medical nutrition therapy (97802โ€“97804, G0270), diabetes and obesity programs.
  • Physical & Occupational Therapists (PT / DPT, OT / OTR) โ€” tele-rehab evaluation and guided exercise programs where state practice acts permit.
  • Speech-Language Pathologists (SLP / CCC-SLP) โ€” tele-speech therapy for pediatric and adult populations.
  • Clinical Pharmacists (PharmD / RPh) โ€” comprehensive medication reviews, MTM, and collaborative-practice services; may also serve as CCM clinical staff.
  • Audiologists (AuD) & Genetic Counselors (CGC) โ€” remote hearing care and tele-genetics consults.

๐Ÿ“‹ What we provide

  • HIPAA-compliant, AI-enabled platform โ€” video sessions, scheduling, discipline-appropriate documentation templates, outcome measures (PHQ-9, GAD-7, and specialty instruments), and patient engagement.
  • Warm referrals from the medical, dental, and coordination network โ€” you are not left to find your own caseload.
  • Credentialing support: state licensure guidance including compact pathways (PSYPACT, Counseling Compact, Social Work Licensure Compact, NLC, PT Compact, ASLP-IC), CAQH, and commercial payer enrollment where your discipline is payer-enrollable.
  • Records ownership: you own and control your complete clinical record and all patient data.
  • Escalation infrastructure: crisis protocols, psychiatric backup through the practitioner network, and 24/7 routing.
  • 24-hour initial training plus annual compliance, documentation, and coding updates.

โœ… Your obligations

  • Maintain a current, unrestricted license/certification in every state where your patients are located (or valid compact privilege / telehealth registration).
  • Carry professional liability insurance appropriate to your discipline covering telehealth services.
  • Practice strictly within your state-defined scope; no diagnosis or prescribing unless your license authorizes it.
  • Obtain and document informed consent (including telehealth-specific consent) for every encounter; verify patient location at each session.
  • Maintain crisis/safety plans and local emergency resources for every active patient.
  • Report licensure changes, sanctions, or claims within 30 days; re-attest CAQH every 120 days where applicable.

๐Ÿ’ฐ How you earn

Per-encounter, roughly $120โ€“$600 per appointment depending on service type, duration, and payer vs. self-pay mix โ€” psychotherapy codes (90832โ€“90837, 90846/90847), health behavior codes (96156โ€“96171), MNT, tele-rehab evaluation/treatment, MTM, and self-pay coaching or program packages. Income scales with the sessions you complete and the panel you retain. The same honest disclaimer applies: nothing is guaranteed; your revenue is a function of your completed encounters.

๐Ÿ—“๏ธ Onboarding & timeline

  • Setup (platform configuration + credential verification) completes within 30โ€“45 days of initiation.
  • Cash-pay and referral-based caseloads can begin within 15โ€“30 days; commercial payer enrollment, where applicable, runs 90โ€“180 days in parallel.
  • Ongoing monitoring of licensure, compacts, and compliance throughout the contract term.
Job 4 ยท For Dentists, Hygienists & Dental Support

Teledentistry Practitioner โ€” Independent Contractor

๐Ÿฆท Role overview

Teledentistry Practitioners extend GoTo Telemed's bridge model into oral health โ€” providing virtual screenings, triage, follow-up, and patient education, and connecting patients into the integrated medical-dental-behavioral-vision referral network. Dentists deliver diagnosis and treatment planning within scope; Registered Dental Hygienists focus on preventive screening, education, and coordination.

๐Ÿ‘ฅ Who this role is for

  • Dentists (DDS / DMD) โ€” virtual consults, triage, treatment planning, and referral to in-person care.
  • Registered Dental Hygienists (RDH) โ€” preventive screenings, oral-health education, hygiene coordination, and monitoring within state scope of practice.
  • Certified Dental Assistants (CDA / EFDA) โ€” support tier: intake, imaging coordination, scheduling, patient education using approved materials, and records management under dentist direction (no independent clinical judgment).

๐Ÿ“‹ What we provide

  • HIPAA-compliant, AI-enabled platform โ€” video consults, scheduling, EHR, and patient engagement.
  • Dental-specific tooling: CDT dental coding and 837D electronic dental claims via integrated clearinghouses.
  • Credentialing support for state licensure and commercial payer enrollment (CAQH where applicable).
  • Records ownership: You own and control your complete EMR and all patient data.
  • Integrated referral network across medical, dental, behavioral health, and vision specialties.
  • 24-hour initial training plus annual coding, compliance, and technology updates.

โœ… Your obligations

  • Obtain and document informed consent; comply with HIPAA and state teledentistry law.
  • Report licensure changes, sanctions, or claims within 30 days.

๐Ÿ’ฐ How you earn

Like every GoTo Telemed track, teledentistry is per-encounter (roughly $120โ€“$600 per appointment depending on service and self-pay vs. payer mix), billed through dental-specific coding. Preventive screenings, triage visits, treatment-plan consults, and coordinated referrals each generate encounter revenue. Income scales with the volume and complexity of the visits you complete.

๐Ÿ—“๏ธ Onboarding & timeline

  • Setup (platform configuration + credentialing) completes within 30โ€“45 days of initiation.
  • Go-live with your first patients typically within 15โ€“30 days of a completed application.

Straight Answers

Frequently asked questions

Is this a salaried job?

No. Every role is a 1099-NEC / 1099 independent contractor position. There is no salary, no W-2 benefits, and no guaranteed income. You run your own supervised digital practice and are paid per encounter. You are responsible for your own taxes, insurance, and business expenses.

Why is there an $890 fee if this is a job opportunity?

Because you are launching a business, not taking a payroll position. The one-time $890 covers state accreditation, malpractice insurance with tail coverage, external compliance verification, credentialing support, and platform setup โ€” costs that normally run into the thousands and take months to arrange alone. You keep the resulting asset: your credentialed practice, EMR, and patient data. On payment you also receive a $5,000 platform wallet credit.

What is the $5,000 wallet credit โ€” do I have to pay it back?

It is a promotional grant applied to your account when you pay the $890. It can be used only for GoTo Telemed resources (advanced training, marketing, platform add-ons). It has no cash value, is non-transferable, expires 12 months after your effective date, and you are never billed for it.

Can I really earn $115Kโ€“$500K+ per year?

That range reflects the potential of an actively managed 200โ€“500 patient panel โ€” it is not a promise. Your income is a direct function of how many encounters you complete and how well you retain your panel. Some contractors earn well into six figures; others earn far less if they work part-time or ramp slowly. Reimbursement also varies by payer and region.

Is the fee refundable?

Fees are non-refundable once GoTo Telemed initiates compliance enrollment or digital setup, and no refunds are issued after your first patient encounter. Setup is guaranteed to complete within 30โ€“45 days of initiation. Only apply if you are ready to commit to launching.

Who supervises the Care Coordinator role?

Care Coordinators work under licensed supervising physicians and nurse practitioners. Coordinators do not diagnose or prescribe; all care plans are reviewed by licensed supervisors, and AI-surfaced recommendations always defer to human clinical judgment. This mirrors the CMS "general supervision" model for care management services.

I'm a Medical Assistant / CNA / CHW / health coach โ€” can I really apply?

Yes. The Care Coordinator role is structured in two tiers. Tier 1 (Licensed & Clinical Staff) โ€” RN, LPN/LVN, certified Medical Assistants, Paramedics, Respiratory Therapists, Social Workers, Case Managers and similar โ€” performs protocol-driven clinical coordination under general supervision, consistent with CMS clinical-staff rules. Tier 2 (Certified Support & Admin) โ€” CNA, CHW, EMT, health coaches, peer support specialists, patient navigators, front-office and billing/coding professionals โ€” owns navigation, education, engagement, and operations, with all clinical questions routed to licensed staff. Your tier is confirmed during credential verification and your scope is spelled out in your SLA.

I'm a therapist, dietitian, or rehab clinician (LCSW, LPC, LMFT, psychologist, RD, PT, OT, SLP) โ€” where do I fit?

The Behavioral & Allied Telehealth Clinician track (Job 3) is built for you. You deliver your discipline's services independently within your state scope and license compacts (PSYPACT, Counseling Compact, Social Work Compact, PT Compact, ASLP-IC where applicable), while the medical network and care coordinators route referrals to you and handle engagement logistics.

I'm a CRNA, CNM, or CNS โ€” which track applies?

Advanced practice registered nurses (NP, CNS, CNM, CRNA) qualify for the Telehealth Practitioner track, practicing within state scope โ€” CNMs in virtual women's health and prenatal/postpartum care, CRNAs in pre-anesthesia tele-assessment and pain management where permitted, and CNS as CMS-recognized care-management practitioners. APRNs who prefer panel coordination over encounter delivery may also elect the Care Coordination track.

My credential isn't listed โ€” what should I do?

Apply and select "Other." Our credentialing team maps equivalent state licenses, national certifications, and internationally educated professionals holding active U.S. licensure to the closest track. If no compliant scope exists for your credential, we will tell you plainly rather than take your fee.

Do I own my patient data and EMR?

Yes. Practitioners own and control their complete EMR and all patient data. GoTo Telemed does not retain medical records beyond what is necessary to operate the platform, and only as a HIPAA business associate.
