You work autonomously while following evidence-based protocols and escalation pathways. You do not diagnose or prescribe; all care plans are reviewed by licensed supervisors, and every activity is bounded by your tier and your state scope of practice.

Following CMS "clinical staff" and incident-to conventions, the Care Coordinator role is structured in two tiers. Your tier is assigned at credential verification and determines which activities (and which billable encounter types) you can perform.
Perform protocol-driven clinical coordination under general supervision of licensed physicians/NPs: symptom check-ins, care-plan reinforcement, medication adherence review, RPM data review, behavioral health integration support, and clinical documentation. RNs may additionally update care plans within scope; LPN/LVN and MA activities are protocol-bounded per state law.
Own the non-clinical backbone of the panel: appointment scheduling and reminders, intake and records collection, SDOH screening and community-resource navigation, health education using approved materials, wellness & lifestyle coaching (self-pay programs), patient engagement outreach, documentation support, and billing/coding operations. No independent clinical judgment, triage decisions, or medical advice โ clinical questions are routed to Tier 1 or supervising clinicians.
APRNs (NP, CNS, CNM, CRNA) qualify for the Telehealth Practitioner track and may also elect coordination work. Tier 2 compensation is per completed task/session at rates below the clinical encounter range; exact rates are defined in your SLA.

This is an additional revenue stream layered on top of your own encounter volume.
