Income is tied directly to the volume and complexity of the encounters you complete — roughly $120–$600 per appointment depending on service type. More panel engagement and more completed encounters mean more income. Slow weeks mean less. That is the trade for autonomy.

The Care Coordinator is the linchpin of GoTo Telemed's longitudinal, digital-first care model — bridging the gap between infrequent clinician encounters with continuous engagement, medication adherence, coaching, and proactive risk management. You work autonomously while following evidence-based protocols and escalation pathways. You do not diagnose or prescribe; all care plans are reviewed by licensed supervisors, and every activity is bounded by your tier and your state scope of practice.
👥 Who this role is for — two tiers, one team
Following CMS "clinical staff" and incident-to conventions, the Care Coordinator role is structured in two tiers. Your tier is assigned at credential verification and determines which activities (and which billable encounter types) you can perform.
Perform protocol-driven clinical coordination under general supervision of licensed physicians/NPs: symptom check-ins, care-plan reinforcement, medication adherence review, RPM data review, behavioral health integration support, and clinical documentation. RNs may additionally update care plans within scope; LPN/LVN and MA activities are protocol-bounded per state law.
Own the non-clinical backbone of the panel: appointment scheduling and reminders, intake and records collection, SDOH screening and community-resource navigation, health education using approved materials, wellness & lifestyle coaching (self-pay programs), patient engagement outreach, documentation support, and billing/coding operations. No independent clinical judgment, triage decisions, or medical advice — clinical questions are routed to Tier 1 or supervising clinicians.
APRNs (NP, CNS, CNM, CRNA) qualify for the Telehealth Practitioner track and may also elect coordination work. Tier 2 compensation is per completed task/session at rates below the clinical encounter range; exact rates are defined in your SLA.

Telehealth Practitioners are the licensed clinical backbone of GoTo Telemed. You conduct synchronous and asynchronous visits, manage diagnoses and prescriptions, and provide the clinical authority that supervises Care Coordinators. You retain sole discretion over clinical decisions and scheduling as an independent contractor — GoTo Telemed supplies the platform, patients, and back office.

Who supervises the Care Coordinator role?
Care Coordinators work under licensed supervising physicians and nurse practitioners. Coordinators do not diagnose or prescribe; all care plans are reviewed by licensed supervisors, and AI-surfaced recommendations always defer to human clinical judgment. This mirrors the CMS "general supervision" model for care management services.
I'm a Medical Assistant / CNA / CHW / health coach — can I really apply?
Yes. The Care Coordinator role is structured in two tiers. Tier 1 (Licensed & Clinical Staff) — RN, LPN/LVN, certified Medical Assistants, Paramedics, Respiratory Therapists, Social Workers, Case Managers and similar — performs protocol-driven clinical coordination under general supervision, consistent with CMS clinical-staff rules. I'm a CRNA, CNM, or CNS — which track applies?
Advanced practice registered nurses (NP, CNS, CNM, CRNA) qualify for the Telehealth Practitioner track, practicing within state scope — CNMs in virtual women's health and prenatal/postpartum care, CRNAs in pre-anesthesia tele-assessment and pain management where permitted, and CNS as CMS-recognized care-management practitioners. APRNs who prefer panel coordination over encounter delivery may also elect the Care Coordination track.
My credential isn't listed — what should I do?
Apply and select "Other." Our credentialing team maps equivalent state licenses, national certifications, and internationally educated professionals holding active U.S. licensure to the closest track. If no compliant scope exists for your credential, we will tell you plainly rather than take your fee.
Do I own my patient data and EMR?
Yes. Practitioners own and control their complete EMR and all patient data. GoTo Telemed does not retain medical records beyond what is necessary to operate the platform, and only as a HIPAA business associate.
